Dell PowerEdge: VLT0204 - System Board PSx PG Fail voltage is outside of range.

Summary: Due to the environment, a false positive or a hardware failure some electrical power is detected on the power ground of a component.

Symptoms

VLT0204 is a common server error message, indicating that the server hardware detects that the voltage is too high or too low. If the voltage is abnormal multiple times, the system may shut down power in fail-safe mode.

 

Diagnosis

1. Get the full error message. There are four common error messages (although more exist):

VLT0204  the System Board fail-safe voltage is outside of range.
VLT0204  the System Board PSx PG Fail voltage is outside of range.
VLT0204  the System Board NDC PG voltage is out of range.
VLT0204  the System Board BP 1.5v PG voltage is outside of range


The error will be displayed over the front panel LCD or the health indicator may blink. Additionally in the System Event Log  (SEL) or LifeCycle Log (LC Log). An additional critical event maybe displayed in the log "The temperature for power supply x is outside of range"

Solution

It is possible that due to the environment, a false positive or a hardware failure some electrical power is detected on the power ground of a component. When this condition occurs, the error messaged is triggered because there should be no power detected on the power ground. The solution is to reset the server power to remove any residual power and to update the firmware to the latest version. If the issue occurs again, a hardware diagnostic has to be started and can in many cases be done by swapping the components to check if the error remains in the slot or is attached to the component.

1. Check the electrical environment, exclude environment voltage instability as the cause of server failure.
2. Update the iDRAC and BIOS to the latest version

3. Try to swap any component mentioned in the error message and update the firmware
-PSx PG Fail: PS means Power Supply, try to swap the PSU1 with the PSU2 and check if the error follows the slot. Try to update the PSU firmware .
-NDC PG voltage: The NDC is the Network Card on the mainboard. Try to reseat the card and update the driver and firmware.
-System Board BP 1.5v PG voltage: Update the BIOS to the latest version.
-System Board fail-safe voltage: Update the BIOS to the latest version.

4. ImportantPerform a power drain. A downtime is required in order to complete this operation as follows:
1- switch OFF the server
2- remove both power supply cables
3- press and hold the power button for 30 seconds
4- connect back the power cables and then power ON the server
5- get into BIOS, select iDrac settings, go to system logs and clear all the previous logs

If the error appears again, there is probably an issue with the component, please contact the Dell Technical Support.
