Dell Latitude 12 (7202) USB Device Not Recognized

I'm unable to get it to work with any USB device.

When I plug in an intelligent device (usb drive / phone) I get a "USB Device Not Recognized Error".  When connecting a phone it will charge for 1-2 seconds until the USM Device Not Recognized Error appears on the screen.  

Does anyone have any experience with this?  Is it just a hardware problem?  I've run self diagnostics and they do not report any problem with the USB port.  I've made sure it's enabled in BIOS, updated Dell drivers, disabled fast boot (many of the google suggestions to this type of problem).  

DAE have any other ideas?
