"Unable to generate CSR" - need more info

Hello,

A client has a very old Dell server running racadm 2.90 Build 04. I can't get into the Web UI because the browser is reporting that the certificate a reused issuer/serial number, which (unlike "mismatched CN" or "cert is expired") is not an error you can override. So, I have to use racadm to generate a new certificate, but when I try to generate the CSR, the only error I get is "unable to generate CSR." I need to know what the underlying problem, is so I can fix it.

I know I'm probably going to have people telling me to update the firmware, but even if updated firmware is still available from Dell, installing it isn't an option right now, for reasons I'm not going to discuss here.

I'm new to racadm, so if there's an obvious solution to this problem, I apologize, but I have no idea what it is.
