Having Trouble creating a volume for T630

1. I have 8 disk

2. 2 are for OS and configured as RAID 1

3. 4 are Data and configured as RAID 5

4. 2 are hot spares

5. I can see 2 VDs in server manager (VD0-OS, VD1 for Data)

6. However when I try to create a volume in server manager the VD1 shows up but I do  not see the VD. Screen shots attached  

Any help is highly appreciated!

Here I can see both VDs in server Manager:

No volume associated with VD1

VD1 doesn't show up when I run create volume wizard
