Are you running Windows 2000 SP4, Windows 2003 or Windows 2003 SP1? [Added:] If Windows 2003, are you using Storport? Are you connected to CLARiiON, Symmetrix or both? You may need to load Microsoft patches.
See Knowledgebase Solution emc135768: Disk rescan may cause disks to disappear and reappear in a Windows 2003 environment with STORPort drivers and Microsoft hotfix (QFE) 908980 installed. The problem occurs in Symmetrix environment with patch 908980, which is storport.sys version 5.2.3790.2571. Does it match your configuration?
Storport.sys is a Microsoft driver. The easiest way is probably to find C:\WINNT\System32\Drivers\storport.sys, Right-click, "Properties", "Version". To see if Hotfix was installed, check Control Panel Add/Remove Programs - it should look like KB908980.
Storport.sys .1830 comes with SP1. The solution I mentioned does not apply, but there have been a number of EMC-approved Microsoft updates to storport.sys. For example, see http://support.microsoft.com/kb/903081 : "An inquiry page 83 compare operation is not performed correctly. This behavior may cause devices to disappear and then to show differently after another scan. This issue may cause data loss because the writes to the original device may fail." Therefore, I would try to go for a more recent storport.sys version. The latest supported comes in Microsoft patch 932755 - http://support.microsoft.com/kb/932755 .
However, this is all forum-based and I did not see your configuration in detail. So you might want to open a case with Microsoft and / or EMC. If you open a case with EMC, you will need to send EMCREPORTS for a more detailed analysis of your environment.
