I have subsequently found out that my sync problem only occurs when I use the NVIDIA High Definition Audio Driver (version 6.3.9600.16384) provided by Microsoft for the HDMI output of my NVIDEA GeForce GT 720 video card.
When I use the Realtek High Definition Audio system of my PC (driver version 6.0.1.7246) with external speakers there is no sync problem.
I, therefore, suspect that there is a problem with either the NVIDIA HDMI sound electronics of the video card or a driver problem with the NVIDEA High Definition audio driver provided by Microsoft.
If anyone can help me resolve this problem so that I can use the HDMI port on my TV to input my PC generated AV signals and have the audio always in sync with the video, as it should be, I would really appreciate it.
I have filled out a "bug fix" report with NVIDIA, a couple of days ago, no answer yet. And who really knows how to contact Microsoft anymore ... and get a satisfactory answer from them without paying for it?
No, really, if anybody does know how to elicit useful help from Microsoft, please let me know and I'll give it a try.
Well, here's a surprise, I waited until the out of sync problem occurred again and, in order to eliminate my HDTV as the problem, I turned my TV off, took the HDMI cable from my TV and plugged it into my wifes HDTV. The sound was in sync! I was quite surprised. Thinking my problem must be my 7 year old Sharp HDTV I was contemplating getting a new one when I hooked the HDMI cable back into the old Sharp, turned the set on and now the audio was in sync on it too. I used the same video file in all of the above cases and did not power down the PC at any time throughout this whole exercise.
So now the question is, does cycling the power on the TV, or unplugging and replugging the output end of the HDMI cable clear the problem? I'm pretty sure it isn't just stopping the video play and restarting it that clears the problem, but as easy as that'd be to try, you can bet I'll try it.
It's interesting to note that when I tried using the Realtek Sound system instead of the HDMI, the sync problem went away as long as I used the external speakers. But, the plugging in of the external speakers disables the HDMI audio output. However, I'm pretty sure that unplugging the external speakers returns the out of sync problem to the HDMI audio channel.
Though things are becoming simpler, I still haven't come to a satisfactory conclusion. Any, constructive, input would still be appreciated.
The Answer ... I think. Since I swapped out my 7 year old Sharp LC-26DV22U for my wife's newer, 2year old, Samsung UN32EH4003FXZA the "sometimes occurring and sometimes not" HDMI audio to video synchronization problem hasn't occurred once in over 48 hours. The longest time between occurrences with my Sharp TV was 8 hours over a sampling period of 33 days.
I used the Sharp as my PC output device with my old Toshiba Laptop at 1366 x 768 resolution for over 3 years without a hitch because I used the VGA input and an external stereo audio cable. When I tried the HDMI interface with the laptop I got a picture but no audio at all so tried the VGA port and that was acceptable.
I got a new Dell desk top a little over a month ago and went straight to the VGA/external audio connection setup but was disappointed with the video. It was nowhere near as sharp, clear, and crisp as it was with the laptop. So I thought, " the specs on this Dell eat the specs on the Toshiba as brief appetizer before lunch" so I tried the HDMI input to see if that worked. It did... sort of the video was noticeably better than for the laptop and the HDMI audio worked!...for a while then after enjoying a few shows I sat back down to watch another and the audio was noticeably out of sync with the video and thus my problem was born.
I have concluded that the HDMI implementation in the Sharp TV is wanting. Although I'm not completely pleased with this conclusion it does, pretty much, appear to be the case.
